School Medications and Forms for 2018-2019

Parents who have students needing medications to be kept at school must fill out a new Medication Release Form and bring it in with the student's medication to the American Academy All School Registration on August 8 or 9, or into the front office any time prior to the start of school. Per district policy, all medication forms expire at the end of each school year. Questions? Email the school nurse (CP & LM, MB)
  • Prescription and over-the-counter medications should be in the original, pharmacy-labeled container or original packaging and brought to school in a clear plastic bag clearly labeled with the student’s name.

  • Medications will only be accepted with the proper forms signed by BOTH the physician and the parent/guardian.

  • Please make sure all medications brought to school are NOT EXPIRED and (preferably) have an expiration date that will last through the end of the school year.

The TDAP booster is required for all Colorado students entering 6th grade.
If your child has not yet received this immunization, please contact your pediatrician and schedule it before school starts. We will be reviewing and collecting immunization records and forms at the American Academy All School Registration in August.
